To attach to the clinic, you will need:

• passport of a citizen of the Russian Federation or a temporary identity card for a child up to 14 years old – birth certificate; Polis of OMS (or temporary policy of OMS);
• SNILS (if available);
• In the event of a change in the place of residence – a document confirming the change of residence;
• If you need to attach a child, a document certifying the identity of the legal representative of the child;
• If you submit an application personally – your application for attachment, if you personally attach a child or a person over 18 years old – a statement of the representative.

Option 1. In online mode
Application for attachment can be submitted electronically in the "Services and Services" section on MOS.ru
Attaching the clinic, by writing an application for the attachment in the selected medical organization or using the service on the portal of the State Service of the Moscow Government https://www.mos.ru/services/ in the "Health" section → "Medical assistance" → "Attachment to the clinic". Please note that to obtain an electronic service for attaching to the clinic, the applicant must be registered on the Moscow portal of public services with the introduction of SNILS.
The term from the moment of applying to attachment and the possibility of remote recording to the specialists of the selected medical institution is 3 days.
You can apply only on your own application. The service is available for users over 18 years old, having a valid Moscow OMS policy.
IMPORTANT: Attach through the MOS.ru portal only to medical organizations, the subordinate health department of Moscow.

Option 2. Personal
The patient or his representative must personally submit a statement of attachment in the chosen clinic.
The clinic who adopted a statement checks the indicated information.
Upon completion of the check of the polyclinic, the patient announces the attachment.

If the child was born before October 1, 2018, it is necessary for an online attachment that the birth certificate is issued in Moscow. If the child was born after October 1, 2018, it can be attached to the medical institution, even if his birth certificate was issued outside of Moscow. Also, in both cases, it is necessary in order to submit an application for an online application that the legal representative goes through parental verification procedure.
